State of Oklahoma: Photography Licensing Agreement & Model Release

This contract relates to an agreement in regard to the use of personal media and images utilized for an artistic or commercial endeavor by and between the following parties:

Photography Licensing Agreement & Model Release

I, [first-name] [last-name], age [your-age], for good and valuable consideration, the receipt of which is hereby acknowledged, hereby absolutely and irrevocably authorize Ken Kniskern and K2ProStudio as well as their legal representatives, assignees, and transferees – to include anyone in related agreements that I may work with as a part of the photo shoot scheduled on [your-date] – to use, publish, reuse, republish, distribute, disseminate or otherwise make publicly available for any artistic or commercial use, including advertising, publication, or illustration images of me and/or my property.

This agreement relates directly to any and all photographs and videos taken with or without my knowledge wherein I am the subject, or where I may be in the background during the photo shoot on [your-date] in whole or in part, whether individually or collectively with any other material, in any and all forms of media now or hereinafter known and/or developed, including, but not limited to, in print media, in digital media, on the Internet, in composite images, or distorted images or for any other lawful use as may be determined by Ken Kniskern and K2ProStudio in their sole and exclusive discretion, including the the name [first-name] [last-name] without restriction.

Release of Liability

I further completely release and discharge Ken Kniskern and K2ProStudio as well as anyone in related agreements that I may work with as a part of the photo shoot scheduled on [your-date] from any and all demands or claims that may arise out of or otherwise be connected with the use of the photographs featuring my image or my property, including, without limitation, and any all claims for the violation of a right of publicity, a right of privacy, or libel.

Proper Attribution of Credit

I waive any and all rights to approve or otherwise review any uses of the photographs featuring me, my image or my property with the understanding that for the period of 2 years from [your-date] that I will receive proper credit as the model in any photograph that was created during this photo shoot and subsequently posted to any portfolio, website, or social media site by the photographer.

As a stipulation to this agreement, I agree to properly attribute credit to Ken Kniskern and K2ProStudio as well as anyone in related agreements that I may work in the creation of any photograph that we receive and subsequently post to any portfolio, website, or social media site for that same 2 year term.

I also agree that I will not knowingly alter or edit the images shared and attributed as documented above including refinements, filters, cropping, etc. and to only present the images as they were delivered to me by the photographer.

I request that any images created in this shoot that are posted on Facebook identify me as the model with the following attribution:

I request that any images created in this shoot that are posted on Instagram identify me as the model with the following attribution:

I will identify Ken Kniskern & K2Prostudio on any images created in this shoot that I post on Facebook with the following attribution:

I will identify Ken Kniskern & K2Prostudio on any images created in this shoot that I post on Instagram with the following attribution:

Terms of this Agreement & Release

This authorization and release shall inure to the benefit of the legal representatives, licensees, heirs and assignees of Ken Kniskern and K2ProStudio and shall also be binding upon me, my heirs, assignees, and legal representatives without a defined end, unless otherwise agreed upon in writing by all involved parties.

I certify that I, being a legally competent adult of full legal age, have read this document thoroughly and understand the entirety of its contents in full. Furthermore, I affirm that as a model in the State of Oklahoma, that I am currently [Representedby], and that participating in this photo shoot will not violate any contract or agreement that might already be in place.

I certify that any personal media or images covered in this release were entered into without force, coercion, or threat whatsoever, and were taken with my full consent. I further agree to hold blameless and free of all accusation of such force or coercion Ken Kniskern and K2ProStudio as well as any person or venue in related agreements that I may work with as a part of the photo shoot scheduled on [your-date].

I fully acknowledge, understand, and accept that this Model Release & Licensing Agreement shall be governed and interpreted in accordance with the laws of the state of Oklahoma and should any provision of this contract be judged by an appropriate court of competent jurisdiction as invalid, it shall not affect any of the remaining provisions in any way, to the extent possible.
